  1. To submit for approval please fill out and print the form below and send it with your ebuy2 request. Your voltage and configuration must be submitted correctly. On ebuy2 please choose CAMS group and Vehicles, Delivery & Industrial equipment, CMC
  2. If you are ordering more than one unit and the specifications or shipping addresses are not identical, you need to fill out a separate order form for each unit that is different.
  3. If you are ordering five or more identical units being shipped to the same address at the same time, call Advance Lifts for special freight rates.
  4. *A surcharge of $500.00 for deliveries to CA, WA, OR, ME, NYC, Long Island and Southern FL counties of Palm Beach, Broward, Miami-Dade, Collier, Lee and Monroe.
